REQUEST FOR SCHOLARSHIP ASSISTANCE

2008-09 Season

Please see the link below for the scholarship application. SCAHA will give priority for scholarship funds to those players who were registered with a SCAHA club last year, who are currently registered with a SCAHA club, and whose need for assistance is greatest. We receive many requests for assistance and cannot award scholarships to everyone who submits an application.

If SCAHA awards a scholarship, the funds are sent directly to the club on your player's behalf. Any differential between your scholarship award and your club dues is your personal responsibility. We do not award scholarships to anyone appearing on the league delinquency list.

In order to be considered for a scholarship, you must do the following:

• Print out a copy of the scholarship application (see link below)
• Copy your 2007 federal tax return
• MAIL (do not e-mail) the application and the tax return to the address listed on the application. DO NOT SEND IT "SIGNATURE REQUIRED"!!

If you do not send the tax return with the application, your application will not be considered.

The deadline for submissions is OCTOBER 3, 2008. Notification of scholarship awards will be mailed the week of November 17, 2008. Scholarship checks will be sent to club in December 2008 and will be applied to December dues. Due to the large volume of applications, we will NOT send letters to those who are NOT receiving a scholarship. If you have not received a letter by December 5, 2008, you will not be receiving a scholarship this season.
